Virginia QR code won’t add to Health records

Yesterday, I logged in to the Virginia Department of Health site (https://vase.vdh.virginia.gov/recordrequest.html) and scanned my QR code as displayed on the site. No luck. I also tried downloading the PDF Record version and scanning that QR onscreen. In both cases, I was getting the same "Unable to add data" error you did.

I then updated my phone to 15.1, and reattempted scanning the QR onscreen on both the VDH site and the PDF Record. Still got the same "unable to add data" error in Health app. Was using a Dell monitor, btw.

Today, I tried scanning the onscreen QR again--using a different monitor; iMac--and it worked. Vax record was successfully added to Health app on the very first attempt.

So if it's not working for you in a prior iOS version, I recommend upgrading to 15.1, and also try some alternate methods of scanning the QR code (maybe even print it out and scan?) Good luck and hope it helps.

My wife is on 15.0.2 and the Virginia QR code works. I am on 15.1 and mine does not work.

If I try to add my wife's QR to my phone, it works.

Based on that, it's sure suspect that some of the Virginia QR's are not properly formed.

Genius! The only thing I didn't try was capturing directly from the big screen. I pulled up the QR (again!) from the Va site on my iMac and bam! it work the first time.

Happy it finally worked, but it shouldn't be this hard.

I updated to 15.2 and tried the old QR code and it did not work. I pulled up the QR again and it worked from my Macbook Air. I agree with Proximus that the Virginia QR's were not properly formed but it looks like they fixed it.

I printed the QR code on an old laser (HP) printer. my iphone has 15.2. But it didn't work at all. The I printed the QR code on an injet printer (old Brother). It worked perfectly. I have no idea why!

Virginia definitely borked something in the QR. It fails the https://thecommonsproject.org/smart-health-card-verifier, which is posted on the card itself. I spent couple hours, working on the decoder. The FHIR data appears to be fine. If validated with no issues

Same. Two of three are fine, the 3rd is invalid. Tried in two apps. I tried using a 5k iMac screen. I also tried increasing the size of the QR code, using the 0.5x, 1x and 3x lenses and different distances, etc. No luck. Pretty sure it is a VDH issue.

I think I have some idea of what is going on. One of the signing certificates is messed up. The QR itself is fine. It doesn't matter if you scan it from the screen or print it.

To make it work, request a record from VA again. They generate the QR for each request. For me, it worked after a third request. So try it again, if it fails, wait a day and request again.

This is happening to me now as well with the booster. Old QR code scanned in, updated to IOS 15.1.1 and also got my booster QR. The QR doesn't verify with verifier as well, and it won't load into the wallet.

I was able to add mine prior to 15.0.2 but get the same error as you when I try and add my booster. Hopefully, they will fix the issue soon!

I am on 15.1, and it doesn't work. One thing - I have it "self-reported" to other linked health records providers. Maybe that creates a problem.

PS: I removed the Inova account, it still fails

In my experience it needed to be iOS 15.1+
