Can't use iPhone Upgrade progam with T-mobile

I've been trying to upgrade my iPhone for the past month. I'm able to get through all most the whole thing but just after confirming with my carrier, I get an error message that doesn't really give any useful information.

It seems like everything is going fine and then this: over and over

Can anyone help me out with this?
