I am unable to add my immunizations to the health app

Thanks for using Apple Support Communities! We see you're not able to add a Covid vaccination to your Health app, and we'd like to help.

The article which you viewed before posting your question, does have the steps to add your vaccine information. You do need to have a QR code or a downloadable file from your health provider.

Add verifiable COVID-19 vaccination information to Apple Wallet and Health

If you have one of the above, and are still not able to add, verify with your health provider that the Health and/or Wallet app(s) are supported.

If so, then you'll want to contact Apple Support.

Make sure to provide what happens when you scan the QR code, and any error messages you received.
