For some reason when I wake my Mac from sleep the screen is black and none of the buttons work. Eventually, I get to the "your computer has restarted because of a problem" then I press any key to resume startup. Does anybody know why this might be happening?

I am on Big Sur and resetting SMC and NVRAM didn't help.

"AHCI" would imply an issue with an internal SSD. Try to check your SSD with Apple Diagnostics if it reports storage device error(VDH002, VDH004).

This extension shows in a position that suggests it has been added manually:

Also, AHT reports nothing wrong with the computer. However, when I take the SSD from my MacBook Air and put it into my MacBook Pro, it doesn't crash immediately after waking from sleep. Is this a a sign to replace the SSD?
Yours is looking more and more like a hardware issue, possibly a problem with the ThunderBolt or PCIe hardware.
If you have not done so recently, you should run the diagnostic. But it rarely finds more than gross issues.
You may want to consider an evaluation at the Genius Bar at an Apple-owned store. If that is available to you, your appointment is free.
The diagnostics found nothing wrong so I'm suspecting it's the SSD on it's way out.
I have another log here. They are all different. One Begins with PRT0::setPowerState and the other begins with AppleAHCIDiskQueueManager::setPowerState.
what does the panic report look like now? you can post it on the forums with the "additional text" icon:
One of the logs started off with "PRT0::setPowerState". Is this related to the SSD as well or is this related to a different issue?
I clean installed and it still did the same thing without anything other kexts installed and it still crashed when waking from sleep.
If Grant is right, you know what this means. Keep a fully functioning back up ready to go.
