With the new iOS 15 on iPhone, Apple Music no longer checks for “song already in playlist” when attempting to add a new song/track to existing playlist. This was very useful in iOS 14, particularly for large playlists where you need to avoid duplicates.
Please investigate and advise.

Apple Support Communities is a great place for help with Apple Music! Being notified if you’re adding a duplicate song to a playlist is important, and is a feature that should be present in iOS 15. If that feature isn’t working as expected try restarting you iPhone.
If restarting doesn’t resolve the issue, please make sure you’re running the latest iOS version. There have been several updates to iOS 15 since the initial release. If you need to reference the steps for updating they can be found in the article below.
