Fell for scam text

I clinked on what I thought was an NHS text and followed a link and started filling in a form. I gave my name address then I clicked continue to next page of form where it was asking for credit card details I which point I realised it was a phishing website. I close the page then cleared history and cookies on safari. I am worried though that the website has collected my name and address on that beginning part of the form. Is there anything I can do like put a warning on my credit file so if anyone try’s to take out an credit in my name they do extra checks? So used to getting alerts from doctors surgery I wasn’t suspicious until they asked for CC details on second page of form

Probably not much to worry about. Names and addresses are readily available.
