Import of markup photos not working, and duplication on import

I've not imported photos from phone to mac in awhile, so the following happened:

#1 importing markup (edited) photos doesn't work

I have a few markup/edited photos, with highlighting/drawings on them. The import screen shows both the markup version and original, which is strange as iPhone only show the markup version. If I try and import one original photo, nothing gets imported. The edited markup photo gets imported fine.

If I try to import a bunch of photos, some marked-up and some not, nothing happens. If I try to import a bunch of not edited photos, it works fine.

A friend's iPhone photos get imported fine, they don't know how to markup on an iPhone, or how to edit photos, so they don't use it.

#2 - duplication of third party photos/video clips

Any photos or clips having to do with any third party apps, Retouch app, Ring, Instagram.. are duplicated over and over on every import attempt. They appear as New, and I'm up to 8 copies of all of them before I noticed. Each copy has the same same filename, stats. Deleted them manually from phone to stop this issue.

To solve the above, as per previous posts, I have tried - create new mac user to try Photos on that, resetting iphone & restore from backup, resetting Location&Privacy settings, Photo app library repair, new blank library. Nothing helped. I'm trying iCloud as an option (set to Keep Originals on Phone), haven't uploaded much.

BigSur 11.6.1, iPhone 15.1, Photos 6.0, ios photos is set to Automatic import (toggle doesn't seem to help) and Camera to HEIC.
