QR code scan won't open health app on iPhone 6s

We have a QR for my wife's covid-19 vaccinations. We scan the code and it sends us to Apple.com's description of the Health App but fails to open the Health app itself (which is installed). So, she cannot add the vaccination record to Wallet. What's the fix?

No option to add vaccine record to wallet iPhone 12Pro iOS 15.02 While adding vaccine record from CA QR code there is no option to save to wallet (only save to health app, which was chosen). From health app there is no option to add vaccine record to wallet, only copy text. What is the exact process with pictures? Does it just pop up? Is there a hidden menu? Do you have to swipe left/right/up:/down? Is there a special setting needed? (health record already enabled and vaccine status already available in health app, but of course there was no option to add to wallet). Does this require deleting the health record and adding from QR gain after (record added from15.02)? Does this require factory reset and reinstall? or is this simply broken without recourse or explanation?
