Calls are often missed due to the ringer switch somehow moving to the off position when no one has been holding the phone. It appears that the internal spring mechanism relaxes and causes this to occur. It is not the user touching it. Apple needs to address this hardware issue.

Take off the case. tape over the switch with PVC insulation tape, put the case back. I've done it for years.
Or put in a feature request to remove this switch, at http://apple.com/feedback
And if you need to silence it to go into church or the doctors office, set up a Personal Focus with nobody allowed to call or message and invoke it from the Control Panel.
