Cannot move Health Record COVID-19 Vaccine immunization to wallet

If you previously scanned a QR code, downloaded a file, or connected with your healthcare provider to add your health records on your iPhone or iPod touch, you can add your stored verifiable COVID-19 vaccination record to Wallet.

I have my Health Records hooked up to my provider (DukeHealth) and I see my COVID-19 Vaccine under Immunizations, but there is no "VACCINATION RECORD" entry under summary and I cannot figure out how to generate a card / add it to my wallet. What am I missing?
