Some icons in Launchpad and Dock are disappeared

Hi,

I am suffering the error that some icons in launchpad are disappeared.

I've tried:

killall Dock
remove *.db in ~/Library/Application Support/Dock
reboot the system several times.

Any ideas? Thanks!
