As KiltedTim says if it is either coming from the provider, which is why I asked if the number was still registered to him, or from the contacts app in the receiving phone, but until you clarify
I cannot be certain of the situation.
Who's name is on the landline account?
i did and there answer was: husband could have changed it before I left him. They didn’t have a date it happened and confirmed he did not have access to my phone.

First search your contacts app for his name and that landline number.
If neither are there then it surely has to be the landline phone company.
To test that, have a trusted friend call your landline from their cell phone and see what is on their phone.
LD- his number is in my contacts but blocked.
The land line number is not in my contacts nor is his number blocked from the land line contacts.

Going into my iPhone twelve to remove his name off my caller ID is not yet solved.
Contact your cellular carrier. Caller ID with name is a carrier feature.
If you mean someone else receives your husbands name when you call them, then their contacts may need updating with separate entries for you an your husband.
It’s a restricted landline for my protection. Not mine not his.
Where is his name appearing, we still have not determined that.
