How to remove a hacker off my iPhone?

regardless of what anybody says iPhones can get hacked. How do I get rid of my jacket on my iPhone? I have changed Apple ID’s . I have set up two way verification and changed passwords and factory reset my phone more times than I can count .. but I still see shortcuts formed I did not create , websites blocked and new events scheduled with information embedded.. apps like music, books, translation, maps, magnification, FaceTime, measurements, notes, reminders, calendar, messages, mail , photos , health and the list goes on that I keep finding unusual activity frequently appearing in ways of tracking different forms of my privacy.. and it doesn’t matter how many times I change password or change Apple ID it keeps showing someone near Dallas Texas logging in under my Apple ID but it never shows any new devices.. and when I try to follow the trail it always ends at either some kind of Wiki platform or GitHub . Com which has something to do with source code.. please can someone help me shake my hacker? Is there something I can download or do to get rid of my problems. I have got nortons malware protection and many different VPN and nothing works and I have updated to iOS 15.5 please help
