The last few days focus (used to be do not disturb) seems to decide for itself what it wants me to hear. Sometimes I will hear a text but not other times. My apple watch will not get the notifications either. This just started happening in the last couple of days. My husband has had the same issue with his phone. What is going on?
iPhone 7, iOS 14
Go to Settings, Tap Privacy > Focus.
Turn off/on shared with Messages. If already turned off, turn on and back off again. Restart both devices.
Also, I had a similar issue with notifications not alerting me, I turned off "Raise to Wake" and did not notice the issue after that. You can try this if you like. Go to Settings, Display & Brightness > Raise to Wake.

For your iPhone, are you on iOS 14 or iOS15?
For your Watch, are you on OS7 or OS8?
For Focus to work, your iPhone should be updated to iOS15, and your watch should be updated to OS8.

For my iPhone I am on OS15
For my Watch I am on OS8
Focus is turned off on all my devices so I cannot understand if while I am holding the phone in my hand I will not get a message notification.
Focus seems to choose for itself
