Focus seems to choose for itself

The last few days focus (used to be do not disturb) seems to decide for itself what it wants me to hear. Sometimes I will hear a text but not other times. My apple watch will not get the notifications either. This just started happening in the last couple of days. My husband has had the same issue with his phone. What is going on?

Go to Settings, Tap Privacy > Focus.


Turn off/on shared with Messages. If already turned off, turn on and back off again. Restart both devices.


Also, I had a similar issue with notifications not alerting me, I turned off "Raise to Wake" and did not notice the issue after that. You can try this if you like. Go to Settings, Display & Brightness > Raise to Wake.
