It's very likely your car is choosing to use the wired connection for audio and bypassing the Bluetooth connection when plugged in. It explains why you can hear audio just fine when only connected to via Bluetooth.
Depending on the model car you have, check for an AUX or "Mode" option to swap between Bluetooth, Radio, and the auxiliary cable. You can test and confirm the audio from Maps in each state.

To help rule out issues with the connection, go ahead and remove your car stereo connection from Bluetooth settings using these steps:
Go to Settings> Bluetooth, tap the information button
next to the name of the device, then tap Forget This Device.
If you don’t see the Devices list, make sure Bluetooth is on.
Set up and use Bluetooth accessories on iPhone - Apple Support
Once you've removed this connection on your iPhone, do the same from your car stereo to make sure they are no longer in the settings.
Go ahead and pair with your car once more, just like it was the first time pairing. The steps here can help with that:
On your device, go to Settings > Bluetooth and turn on Bluetooth. Stay on this screen until you complete the steps to pair your accessory.
Place your accessory in discovery mode and wait for it to appear on your device. If you don't see it or aren't sure how to make it discoverable, check the instructions that came with your accessory or contact the manufacturer.
To pair, tap your accessory name when it appears onscreen. You might need to enter the PIN or passcode. If you aren't sure what your PIN or passcode is, check the documentation for your accessory.

I unpaired the car and iphone. Had each device forget/delete connection. Re-paired and tried navigation again. I only get verbal directions over bluetooth. The instant I plug-in cable to charge, I no longer get verbal commands.

The iphone is connected to the car via bluetooth. I can play music through siri & apple music but I haven't tried listening to music with the phone plugged-in via the wire. I know the siri directions broadcast over the stereo speakers via bluetooth, but when I plug-in the cable to charge the phone, the verbal directions stop. When I unplug the cable, I get verbal directions again over the stereo.
