Where are you seeing this message, on the phone or on the TV?

I've started to experience this issue as well. it seems specific to the phone based on my testing with other iphones. The accessory firmware it's referring to is the HDMI adapter (the official apple one). Is there any support for this?

The message is coming from the iOS device. The image the original poster listed is the same one the pops up each time you insert the lightning connector into the iOS device. Rendering it unable to play the media on the screen. This message does not show up on another iOS device we have and plays content as it should.

Yeah, it has nothing to do with “TV” Firmware. it’s phone and iOS specific.

No, it’s the dongle, the adapter. Not the tv. Tv doesn’t even have to be plugged in to get the error just the adapter.

I understood the screenshot, you left out the fact that it displayed that message even with the TV unplugged.

No, it wasn’t left out. Original poster provided screenshot. Not sure why you didn’t understand, but that’s cool. I know you’re just trying to help. Personally I’ll wait for next software update as it works on my other devices with older iOS versions.

Then it's refering to the device that is connected to it which would be the TV. The phone wouldn't call itself an accessory.
