When I send a message to my girlfriend, it shows up as being sent by my mothers contact on my girlfriends phone. When I get sent a message it comes from her number, but when I reply to that message on my phone it shows as her having a conversation with my mothers email address. It will show up as 2 individual chats on her phone and 1 on mine. There is no easy way to explain it and yes, it is just as confusing as it sounds.

Yes. It says “My Card.”
Have you checked the Contacts app to make sure your card is marked as Your (My) card?
You all have your own Apple ID, correct?

Yes. I do.
On other peoples phones as well. Including my mothers. Apparently it’s her other one? But I have no links to that ID at all…
