Generic icons appearing in Mail attachments
Since upgrading to macOS Ventura, a number of attachment types such as PDF attachments in Mail are appearing as generic blank icons. How do I resolve this problem?

Give this a try: boot into Safe Mode according to How to use safe mode on your Mac and test to see if the problem persists. Reboot normally and test again.
NOTE 1: Safe Mode boot can take up to 3 - 5 minutes as it's doing the following;
• Verifies your startup disk and attempts to repair directory issues, if needed
• Loads only required kernel extensions (prevents 3rd party kernel/extensions from loading)
• Prevents Startup Items and Login Items from opening automatically
• Disables user-installed fonts
• Deletes font caches, kernel cache, and other system cache files
NOTE 2: if you have a wireless keyboard with rechargeable batteries connect it with its charging cable before booting into Safe Mode. This makes it act as a wired keyboard as will insure a successful boot into Safe Mode.
Sadly true in my current experience. I've tried to remove and reload the icons. No luck. They are correctly displayed in a folder (smart folder) I have on my dock. Interesting they will not display correctly as single items on the dock.
13.2.1 on a MacBook Air M2, same issue. Tried rebuilding the mailbox, and safe booting. When that did not work I clean installed from a USB installer. Did not fix. This is terrible. Makes me think poorly of Apple.
I am having this issue but I do not want to spend 3 hours on the phone with tech support to fix my icons. It looks like crap but I guess I will have to deal with it until they come up with a fix. How long did the entire copy and create new user process take?
Thanks for the update. That is odd though - .docx and .pptx aren't affected for me ; and I thought others too. Do you have powerpoint and word installed?
.pdf we all know about - doesn't matter what is installed.
It is cosmetic - but then so are Apple.

I verified the problem persists with 13.1 update as I previously posted. Also this is true with JPEG and PNG documents.
Yeah, the bug persists with 13.1. Alas.
The blank icons persist in Ventura 13.2 as well...

Thanks for the hint. I also knew about this and tried it before. No luck. I just tried a few more times to see if it would help in the most recent Ventura update. No luck.
I am having this same issue. Started in 13.1, continuing in 13.2

Yeah, I’m confirm that in 13.2.1 problem is still here.
This is such an obvious bug. Just sort of sloppy.
Can you provide us with the steps that you took to resolve issue? And I will ask why this problem occurs on new installation without any old data on new machines?
