On the column display of Mail in Big Sur on a MacBook Pro, I cannot insert a column of flags.

Control-click on the header of any of the columns, and check the box for "Flags":

I’ve managed it. Hold ctrl and click in the top right corner of the column display window. A short drop-down menu appears, including Flags. Click on it and a new column appears on the left to display flags.
