Child no longer has to ask for permission to buy apps, even though it's still setup to require it

We have been using apple's family sharing and "ask to purchase" feature for a few years now. It still works well for my 8 year old, but just recently, after I approved a purchase on his iPad, he no longer has to ask for permission to buy anything. He bought tokens inside games, bought and installed apps, all without having to ask for permission. I looked at his settings under my account, and according to it, "ask to buy" still indicates "require purchase approval" as selected. I toggled the setting, I restarted his iPad, and still it allows him to buy anything he wants without having the "ask permission" window come up or any notifications sent to me. Please Help!
