Health App: High and Low Heart Rate Notification alerts not triggered because devices shown as 'None'

I have tried with Apple Watch and Garmin watch to set up high and low heart rate alert notifications but in the Devices section I cannot add nor select any device linked to the Health app. The devices appear for other data capture components eg heart rate, steps etc so this shows that the link between the app and the devices is set up correctly. To me this looks like a bug in the current version of the app and I have a service ticket with Apple to confirm this. I have seen other posts here with the same issue.

I am wishing to use this feature for health reasons (not aFib) so it is quite important to be, and the main reason I bought the Apple Watch.

Notes:

Apple Watch 8 (iOS 9, up to date)

iPhone 8 (iOS 16.3.1, software up to date)

I often receive high heart rate notifications due to my supraventricular tachycardia (a.k.a. Wolff–Parkinson–White syndrome). Lucky for you, I feel like I'm quite qualified to help you get this working. Just bear with me as we compare our phone/watch settings.


Open the Watch app on the paired iPhone.

Scroll down to and click on "Privacy".

Is the Heart Rate toggle green? It needs to be.

Go back, scroll down to Heart and click on it.

Is the toggle for "Irregular Rhythm" green? It needs to be.

It will not trigger a notification if you are active, you need to be inactive for ten minutes then it will check the heart rate (or if you get above the set heart rate it checks if you were active in the last ten minutes - same logic)

Basically if you are sitting in front of the TV all night and your heart goes above the limit it should notify you.


That's why I suggested setting a low rate to see if you trigger that.

Personally I rarely go above 55 on the TV sofa


As soon as you mentioned epilepsy I was remiss in not advising you to reach out for Apple Support and their health team.

Using your watch, open Settings, scroll down to and click on Heart, scroll down to and click on "High Heart Rate Notifications"

Where is the checkmark? Mine is on 110 BPM.

Go back then click on "Low Heart Rate Notifications"

Where is the checkmark? Mine is on 40 BPM.


Since 2019, I have 181 recordings for "High Heart Rate"


In the screenshot in my original post, you can see where the data was coming from.

"Jeff's Apple Watch" was a Series 3 watch.

"Jeff's Watch 8" is my current watch (Series 8)


In all that time (2019 to now), I've never had a "Low Heart Rate" event notification. Likewise, in the Data Source of the Low Heart Rate screen, there aren't any devices listed.



The "High Heart Rate" notifications only happen when my watch determines that my heart rate has been higher than 110 BPM, for at least 10 minutes, and while I don't appear to be active.


This is the conclusion that I'm coming to:

Although things are setup and ready, I don't believe that you have had any episodes that meet the requirements for a notification (heart rate at or above the BPM setting that has lasted for at least 10 minutes while inactive). Until these conditions are met, there isn't going to be a device listed in that section of "Data Source" (like there isn't anything listed for my low heartbeat Data Source).


If Apple comes back with a different conclusion and/or even gets a device to show up in that field, please PLEASE come back to this thread and let us know what they did so that we can help others more efficiently.

The "none" applies to app privacy not the watch privacy. Mine says the same thing. I have no third party apps that want to read or write to my my heart data.

If you set your notifications as I outlined above in Watch app settings to relatively low "high rate" and relatively high "Low rate" then you can see if it triggers a notification. Then set them to the real desired levels.

It will show as a data source when it provides data. You cannot add a watch as a data source manually, it will add itself with the first notification.

I tested it as I suggested, I put in (for me) an artificially high "low rate" of 50bpm which I know I will drop below in a typical afternoon snooze. Darn thing woke me up!


My "Peter's Apple Watch SE" now shows up but my previous watch doesn't show as it has

never triggered this particular warning.




Other health data shows older Watch too

Finally a High Heart Rate Notification came through.

I can confirm that devices then show in the settings.

I can also confirm that the alert is only triggered when the heart rate is over the threshold for 10 minutes, after a 10 minute period of inactivity, and the wearer is not moving (recordings are measured as ‘sedentary’ or ‘background’).

Thanks for the reply.

Can I pick up on something you wrote that has been on my mind. You mention about having a heart rate higher than the threshold for 10 minutes after a period of inactivity. This is something that has puzzled me because the information on the watch / app settings is written in two ways: a ‘heart rate higher than the threshold for 10 minutes’ and ‘a heart rate higher than the threshold during a period of 10 minutes inactivity’. This is a subtle difference but an important one.

I want the watch to trigger an alert for the latter as I am monitoring for epilepsy where there is a 2-5 minute rapid increase in heart rate e.g during sleep. I thought it would be odd if the heart rate trigger was for a high rate for 10 minutes as that’s a long time.

I think this is possibly the right answer here as I’ve seen similar reports for cardio and walking settings not being correct until a workout has been done.

I’ll report back when I speak again with Apple.

My watch is set up as you said but no notifications are generated. Using the Health App is useful to view / check if any notifications are generated, as the watch app doesn’t show them.

What are your thoughts on whether the high heart rate alert is triggered when the rate is above the threshold for 10 minutes OR after a period of 10 minutes inactivity??

And yes, LD150 is exactly right about the couch and watching TV.

That's when I often get the notifications. I also get them while in bed before falling to sleep and in the middle of the night while I am asleep.

Although there is an open heart surgery fix for SVT, I'm not too concerned about it because most of the time I bring on the high heart rate myself due to stress and just thinking about everything on my plate for work (self employed).

Rather than open heart surgery, I probably should just go find a job and work for someone else 🤷‍♂️


I have recordings for heart rates of up to and just over 180 BPM (not pictured). The Series 3 watch is how I found out about it. I used to feel it but thought that it was just heartburn. After getting the Series 3 and being notified, I figured out that the feeling wasn't heartburn 😂

How did you do that, a "test notification"?


Now that you got that, do things show up in the Low Heart Rate screen?

For instance, a record of that? And also, does your watch show up as a Data Source lower on the screen?


If you look at the screenshots that I added this morning, you'll see that I don't have any "Low Heart Rate" records and as such, the "Data Source" field is blank on that same screen.

DrMattBishop is merely trying to get his watch to appear as a Data Source for the High Heart Rate screen. My guess is that there isn't a way to program it, it's something that will be filled in whenever a High Heart Rate is finally detected and logged (of course while being inactive for at least 10 minutes).

Quick follow up question.

I’ve just finished my latest call to Apple and this person said that for High Heart Rate Notifications the heart rate has to be over the threshold FOR 10 minutes AFTER 10 minutes of inactivity.

What is your experience? There is ambiguity in the Apple documents but I thought it was likely that the rate had to just go over the threshold to trigger the alert, not go over for a whole 10 minutes.


I seem to be getting nowhere fast but thanks for your thoughts.
