For the past 3 days, all incoming calls - including contacts - are displayed "No Caller ID." I have not changed any settings; have rebooted, have confirmed software is up-to-date.

That means that your carrier is not including the CLID string when sending calls to your phone. Contact your carrier’s tech support.
