Hand washing reminder when home

What we'd recommend is checking out the requirements that are listed at Set up Handwashing on Apple Watch Ultra - Apple Support. You've done right by checking your location settings. Per that article, make sure that, "To receive handwashing reminders, you must set a home address in your My Card in the Contacts app on iPhone."

I had the same issue after iOS 15. I have found a solution that has worked on my Apple Watch 8 WatchOS 9.6. The problen has been that "Handwashing" was missing under "Location Services" in the iPhone Settings> Privacy & Security> Location Services list.

I turned the "Location Services" toggle at the top of the page off, then turned it back on again. After I did this "Handwashing" instantly appeared in Location Services and it was set to "Always" Now I receive a reminder when I come back home to wash my hands.

It sounds like you're using the correct device and have the correct settings enabled. You can try to resolve the issue by restarting both devices.
