I’ m in Madagascar and I’ve just bought my watch serie 8.
My phone & watch have been updated at the last iOs version.
when setting up the ECG, I get the message : ECG app is nor available in your region. However, Madagacar is into the list where ECG can work.
Need help please.
ECG is
viewed as a medical device which mean it require official approval from the
government of a country before it's legal for apple to offer
these are the countries which has provided with said approval so far

ECG
implies that citizens' medical data be stored by a foreign third-party company
without individual approval by the government of said country, and that is not
something all countries feel comfortable with.

As Billben81 correctly points out, Madagascar is on the list of approved countries for the ECG app→watchOS - Feature Availability - Apple
So one possibility is the phone’s region is not set to Madagascar.
App developers may not develop all apps for all countries.
