The fan on my 2019 iMac is running continuously. I have reset the SMC multiple times. Checked the Activity monitor and nothing looks out of place (Idle is 95%). The fan is so loud that it disrupts workflow. Any advice would be appreciated. This OS is Catalina 10.15.7.

One undocumented cause of hot running in iMacs made after 2011 is the tilt position of the screen. If it is tilted fully down (example, computer placed above your eye line) the top portion of the stand moves too close to the poorly-place exhaust vent:
Like a party trick, I can produce overheat heating at will by tilting the display forward. Oh, the sacrifices made at the Altar of Minimalist Design!

The poster removed the debris from under the air intakes and all was well. One needs to preserve that spacing between the intakes and the desk surface

Since you already checked with the Activity Monitor, and have reset your iMac's SMC multiple times, the only other area to be looked at would be proper ventilation around your iMac to allow it to stay "cool."

You have not considered to upgrade to Mac OS Ventura?
I have a ,iMac 2017, running Mac OS Ventura and i have not some Fan issues!
But you might have a reason not to upgrade your 2019 imac?
