This issue has been going on for nearly a year, through various updates. It is extremely frustrating.
When phone and Mac are connected, two things happen:
Phone: Trust / Don't Trust message keeps flashing on and off and a sound beeps.
Mac: The phone shows up on the sidebar but it is also flashing on and off.
USB cable has been replaced.

It appears you have an issue with your iPhone connecting to your iMac. You mention the "trust/don't trust" message keeps popping up and you see the iPhone appears in the sidebar on and off. You also mentioned replacing the cable connecting them, which is a good first step.
Something else to test to further narrow down where the issue is, is to connect your iPhone to a different port on your iMac. The goal will be to see if it's a specific port, your iPhone connection to your iMac in general, or something else. If you have another device you could test connect to your iMac in the same manner as your iPhone that would also further narrow down where the issue is.

Thank you for your response. I can say that it is not the cable or the port as they charge / connect my wife's phone. I've used a hairdryer to blow into my phone's connection in case there might be be dust but that did not make any difference.
To be honest, I think it's a permissions thing, Apple have been releasing software updates I am tempted to believe it's something to do with that. It never happened when I did not check "Automatically Update".
