Are both your iPad and iPhone signed-in to iCloud with the same AppleID? Have both devices been updated to the most recent version of iOS/iPadOS supported by each device - iOS/iPadOS 16.5.1 and 15.7.7 being current?

Have you enabled iCloud Notes on both devices?

Settings > [Your Name / AppleID] > iCloud > [Apps Using iCloud] Show All > Notes > Sync This iPad | iPhone - set to ON
