My iPhone 12 which I purchased 9 months ago overheats when plugged into any charger and chord combo I’ve tried a lot of different options, it also only seems to drain the battery from a full charge in about 4 hours with minimal use, also in the last 3 weeks the screen has become slower and slower to respond. Looking for any suggestions before I take it to my provider

Unlikely that the provider will be able to help.
Make an appointment at the Apple Store or take the phone to an authorized Apple Service shop to have them hook up the diagnostic equipment and run the tests.

Make an appointment at the Genius Bar of your local Apple Store. It should be under warranty.
