Unable to Add Healthcare Provider to Apple Health

My provider is on the list of companies that allow connecting to Apple Health, and I previously had this provider connected but that connection expired in February.


I am now trying to reauthorize for another year, but I keep getting an error message that says "Unable to Connect - Try adding an account later."


I've been trying periodically for weeks, but don't seem to be getting anywhere. What could be causing this problem?


Steps to Error:

1) Tap on Apple Health/Browse/provider name.

2) Tap on Sign in to Upgrade.

3) Log into provider account.

4) Consent to Apple accessing my information and choose a period of time.

5) Error message.

PS: The tag seems to have my old iPhone. I am using a 12 Pro Max running iOS 17.0.3.
