1: we can’t update our iPad through hotspot from iPhone
2: it is not possible to update Apple Watch gps / cellular models through iPhone to which it is connected using cellular data on iPhone. ( why you need a separate Wi-Fi for the same)

could be your carrier that blocks some of the ports when acting as a hotspot some only allow port 80 (the port one use for web browsing) so that could be an explanation, but there are some posts about latest ios version broke access to ftp using an iphone as hotspot which they could before so that could also be the bug
if it's an apple design decision then you can give them your opinion using their feedback channel
