i Mistakenly approved this app. I can go to her phone and delete the app. She can then go straight to the App Store and download it again. The App Store does NOT send me a notification to approve.
is the only solution to setup a brand new iCloud account? There’s nothing I can find online about how to solve this problem. PLEASE HELP

Indeed, this is expected behavior. Unfortunately, once you have approved the request, there's no way to revoke permission. Your best bet is either to limit the app by changing its screen time limit to 1 minute (there's no way to make it zero) or disabling the ability to download new apps.
