Set ringer volume

When face id and attention is disabled and ringtones and alerts are set not to change based on volume buttons, how do we set the volume for ringtones and alerts?

Face ID is attention aware, so you can glance at an alarm or ringer notification to instantly lower the volume." I consider this a "safety issue for the elderly". Any ETA for a resolution?

Ringer doesn't go off until face recognition is unlocked My ringer never goes off for things like text messages and app notifications until I put my face to my phone and the face recognition unlocks my phone. I have the Iphone 11, my phone isn't muted and do not disturb isn't on. It will very rarely ring with my phone being unlocked but 95% of the time I don't hear my ringer go off which, if you can't tell, kind of defeats the purpose of having the ringer on in the first place. I've searched both the internet and my settings and can't fix this and it's EXTREMELY annoying.

We feel like we've tried everything but cannot get the ringer volume to adjust up to an acceptable level. Sometimes, it just goes to voicemail and we cannot hear the ring at all. Also, the one time we got it to work (at an acceptable volume) it rang ok then quickly dropped to a lower volume. Our research indicates that this may be a built-in feature that comes from the facial recognition option on the phone. Please understand, my wife and I are transitioning from I-Phones 6s and 5c respectively. So, it' obvious, we need help.
