I am trying to set up an alternative appearance for my husband on my IPhone 12 Pro Max. I went to Settings/Face ID & Passcode, entered my password but the option to set up an alternative appearance is not there.

That would mean that you already have an alternate Face ID set up. There is no way to remove just the alternate Face ID, so you will need to choose Reset Face ID and set yours up again, then you will have the option to set up the Alternate Appearance.
