What does Silence Notifications mean on an iPhone?

In Do Not Disturb/Allow Notifications/Options there is a setting for Silence Notifications which is a choice of either Always or While Locked. The description says Incoming calls and notifications will be silenced while the iPhone is either locked or unlocked (for the Always setting). That is incredibly confusing. It sounds like if you choose Always, you will never get rings or tones for texts or phone calls. If you choose While Locked you won't get them when the phone is locked in your pocket. I am guessing this might apply only to the when Do Not Disturb is active but you already set what apps or people are allowed to contact you when Do Not Disturb is active in a different screen.
