iMac Photos won't import iPhone photos in Normal Mode

My iMac 27-inch, Late 2015 running Monterey cannot import photos from iPhone 13 pro running iOS 26.2. When the iPhone is selected in Photos, it doesn't show any new photos (since last import) and shows a somewhat random number of previously imported photos. When I start up the iMac in Safe Mode (shift key on startup), the new photos do appear and can be imported as usual. I've since deleted all of the applications in Login Items. What else can I do to restore the iMac's ability to import photos whilst in Normal Mode?

Maybe some 3rd party app intervenes? 3rd party virus apps and "cleaners" are the usual suspects. An EtreCheck report might provide more clue (set Full Disk Access for it):

Getting rid of Login items was a great response to the Safe Mode analysis! Clearly something else is going on in your User account on the Mac. As Matti Haveri suggests, virus apps and "cleaner" apps are often responsible. EtreCheck may give you some clues.

You could also just make a new User and transfer stuff over, then delete the original User.

With new Silicon Macs PRAM resets are a thing of the past. Maintenance like that is done with a re-boot. But for Intel Macs

And for the System Management Controller, SMC:

However, since the Mac works in Safe Mode, resetting these probably won't change anything-- but it doesn't hurt to try.

I think you ought to try setting up a new User account-- that separates you from all the old garbage. If storage becomes a problem, you can just delete the old user and be done with it. It's probably a good idea to make a good backup before any big changes.

To help troubleshoot your Mac, I would suggest that you consider posting the resultant EtreCheck report for us to review. (Note: The EtreCheck report does NOT contain any personal information.)

Thank you Matti and Richard.

I ran EtreCheck and as you might expect, there is a long list of uninterpretable stuff in the Minor Issues. The only Major Issue is "This computer may be considered obsolete." So one theory is the latest iPhone system update made it incompatible with my older iMac which doesn't accept new OS updates any more. The timing is about right but as is typical, I can't say for sure when this feature stopped working vs when I updated my iPhone OS.

I've gone in and deleted some Library items that were listed in the Minor Issues but I don't know how to really uninstall stuff that has populated multiple system directories with all kinds of junk. It also tells me I have Heavy CPU and I/O usage which I'd love to address if I only had a clue as to how.

Do these computers have a PRAM reset or a way to clear the cache? I know I used to do this back in the day but don't know the steps or if there are steps with this hardware/software.

Thanks again!
