The “Notify Anyway” option isn’t about who’s in your contacts—it’s about your Focus settings. Specifically, it appears when Share Focus Status is enabled.

With Focus Status shared, anyone who messages you—whether saved in your address book or not—can see that you’ve silenced notifications. They may then be offered the Notify Anyway button, which lets their message break through Focus mode.

Bottom line: this is a Focus feature, not a contacts feature. Your address book doesn’t get a vote—your Focus settings do. 😄
