I found the remarkably simple answer to this problem (the side switch on the phone was set to silent)on my phone at another thread:

Hello Melissa,
Here is an article about troubleshooting the phone being silent. Called iPhone: No ring or sound from built-in speaker it can be found here http://support.apple.com/kb/TS1205.
Also I would recommend looking at the side switch on the phone. Here is an article that outlines the sideswitch:
iPhone Buttons
http://support.apple.com/kb/ta38625
Flip the Ring/Silent switch to change between ring and silent modes.
When set to ring mode, iPhone plays all sounds. When set to silent mode, iPhone doesn't ring or sound any alerts or sound effects (alarms set using Clock do sound). By default, when you get a call, iPhone vibrates whether it's set to ring or silent mode.
If iPhone is set to ring mode, you can silence a call by pressing the Sleep/Wake button or one of the volume buttons once. Press a second time to send the call to voicemail.

Thank you SO much. Spent hours triple-checking my settings, but had no idea about the silent switch to the side of the phone. My wife and I now (finally) got our iphones to work properly. You're a star.
My wife had the same problem on her iPhone 6. I checked all the settings and everything appeared to be set where it should be, but still no ring. No moon symbol in the top right corner of the screen. I rebooted the phone (power off, wait about 15 seconds, power back on) an it works fine now.
Maybe these are too obvious, but have you switched the ring/silent switch to silent?
Do you have the volume setting high enough in Settings > Sounds?
Have you tried another Ringtone in Settings > Sounds?
Also, a reset seems to cure many ills. Maybe you should try this as well.

I Had the same problem with my IPhone 6. It took me a little while to figure it out. I went through all the settings over and over again thinking maybe I was missing something. i even reset all setting, called sprint and followed their advice. I followed most of the suggestions I found online. Nothing seemed to help. Finally just out of curiosity I turned off my LTE now everything works fine. A let it rest a day, then enabled LTE again. And calls quit coming through. It seems on my end to be a network problem or firmware problem. But my phone works good in 3G mode only.
On my iPhone 6 with the same issue, I had to turn off the Repeated Calls parameter. It is supposed to prevent ringing when receiving multiple calls from the same person within 3 minutes. It is one of the Do Not Disturb settings.
I dunno if the LTE will make a difference to me, I just flicked the option - but if anybody calls me this time of night,my wife will kill them.. and torture me for weeks.
I have a brand new iPhone 6, worked just spiff for the first week. Then, yesterday, I found messages and missed calls. The phone was in my shirt pocket in a quiet environment, so I didn't just miss them. Later, I was actually looking right at the phone when a call came in - with no ring tone.
No, Do Not Disturb is not active
No, the mute switch is not set to mute
yes, the volume is set high enough to be heard (in the next county)
Yes, i can play music, ring tones, etc just fine. Ergo, hardware seems to be eliminated. I've spent the past 6 hours poking and tinkering and reading the net, and can't find a definitive reason.
THANK YOU!!!! Finally my problem was solved!! 🙂😁

Just to pick a nit: the setting to which you refer is designed to make noise if a second call from the same number comes within three minutes. That is, you turn that feature on, and if I call you twice in three minutes, your phone will ring.
Thank you. I had checked everything else and this was the problem. Easy fix. I didn't think to look at the Do Not Disturb because I don't use that feature but had recently gone through all of the settings to turn off the ones running in the background to save on battery life.
This was really helpful. I had no clue what that moon icon was and the phone rang on occasion. I thought it was the network, but only turned out to be this Do Not Disturb sign. Once I turned it off, I was good to go.
I had this problem too and then realized that the side switch on my iphone was accidentally set to SILENT. Duh. Check the little switch on the upper left side of phone, right above volume keys. If it shows RED, the sound is off. Push it to green and you're all set.
I had the same problem, what fixed it was going into settings>general>reset>Reset Network Settings. My phone rebooted (I did have to re-connect to my WiFi) and boom, it all started working again!
with this problem, my mom had no sound, and would receive messages with sound. if this happens, just press and hold the top power off button, and slide to power down. restart the phone, and it should work again!
Had this issue, everything looked right: DND was not on, volume was on max, plugged in headset to see if sound was coming over that port but found nothing. Having learned the Microsoft method for "fixing" issues, I tried the same "reboot" long-shot to see if that would fix it.
I rebooted (hold power until is shuts down, wait a few minutes, hold power to turn it back on) the iphone. When I turned it back on, it was then working correctly.
My iphone 6 doesn't ring when a call comes in.
