unknown error occurred (-54)

Whenever I try to sync my iPhone 5 I get the following error code:

"The iPhone "Nomad I-phone" cannot be synced. An unknown error occurred (-54)"

I have the latest software updates installed on both my pc and iPhone. I have run the diagnostics program and it does not return any errors.

anybody got any ideas?
