How do I make Forms in numbers app

I am still finding instructions on how to add an input form to a numbers table. The user Guide and other articles tell how to get a Form to appear in Numbers. However, when I tap the "+" in the upper left of the sheet as instructed, all I get is a new sheet.

Do you have the table to link the form?

The table must have at least one column with "data category" name set in the header row.

Then Numbers should show choice of whether new form or new sheet, when you tap [+] on top-left of the sheet.

Create a table with a header row listing the data categories you want to enter in each record, and a header column listing the record names (optional).

2. Tap + (the Add Sheet button) in the top-left corner of the sheet, then tap New Form.

If your spreadsheet includes several tables, a list of tables appears.
