iMac's getting slow May I need to change HDD to SSD ?

I had a iMac (27-inch, Late 2013) which one was working smooth after I installed High Sierra , everything is getting slow . I formatted and re-installed Mac OS High Sierra is same . Downgrade to Mac OS Sierra is same as well.

Im guessing My HDD need to change or have bad sectors or something else . Could you help me about it ?

The HD is failing, that is quite clear from the report. Before you do ANYTHING ensure your backup works and then replace the HD with either a SSD or another HD. One option is to get a USB 3 or even better Thunderbolt enclosure and install the replacement drive in that and then boot from the internal HD (before it totally crashes), format the new drive and then use Migration Assistant to migrate from your bootable clone backup or Time Machine to the new external drive, then set the external drive as your startup drive and you're set. This plan prevents major surgery on the iMac which is not intended for a user to do.
