Add iPhone as Data Source for Health App

The Health app has my old iPhone as a data source. I did have my IPhone X as a data source but I accidentally deleted it. How do I get it back? The older iPhone listed (iPhone 7 icon) is not needed as I meant to delete that one.

How can I delete my old iPhone from the health app? I got a new iPhone and I can't delete my old one from the heath app and this is not letting me sync to another app that I have. I've tried to delete it but it doesn't give me an option to remove my old phone.
