My paper size is set to US Letter, 8.5x11". In the print preview, it looks correct but when it prints, the entire page is printed in the upper left corner.
I have an iMac, OS High Sierra 10.13.6.

Only thing I can think that would do that for 2 different page sizes is if you have selected 4 up in the Print Dialog:
Go to Canon's website and see if you can download a Printer Driver for High Sierra.
What is the actual size of the desk calendar and does it have a second page ie it folds?
The document is scaled. The entire document is re-sized and printed in full in the upper left. If the sheet of 8.5x11" paper was divided into quarters, the entire document would fit in one quarter, in the upper left.
Any thoughts? I'm having the same issue still, whether I print an 8.5x11" page or a 4x6" photo - the entire document/image is printed in full in the top left quadrant. I have restarted Pages and my printer multiple times. It doesn't happen in any other application.
I downloaded the most recent printer driver, closed out everything and printed - same results. I cannot figure this out!
The calendar page size is 5x7". It doesn't fold - rather, each month has an individual page. (It's meant to be displayed on a small easel or stand.)
I'm not printing it to a larger piece of paper. I'm printing a 5x7 to a 5x7" sheet. Same with 8.5x11 and 4x6. It doesn't matter what size paper is in the cassette of the printer, it still prints the entire contents/text/image to scale in the upper left corner.
The issue does not happen when I print from Safari or when I print a Word document.
I am having the same problem printing from Chrome, ios High Sierra 10.13.6, and I am printing a Google Doc. Really weird it was printing fine this morning.
Then if it is the same problem, it is the same answer.

Your preview looks correct.
If you are printing it actual size to a larger piece of paper then I think you can expect it to print to one corner.
Can you explain your issue more clearly.
Are you saying the document is shifted or it is scaled to a smaller size?
Remember we can not see what you are looking at.
You have told us your Mac, your OS but what about what version of Pages?
