I have the series 4 apple watch with cellular and would like to pair to my car so I can take calls on the car hands free and also play music through the car stereo when I go out without my phone.
The car stereo shows up on the watch as an option to pair to but the connection always fails. How do I pair the watch with the car stereo?

You cannot. The Apple Watch does not support pairing with car media centers.

Do apple read these forums? If so, can you please fix this issue as I love the series 4 watch however, not pairing to a car stereo is a major flaw in your software.
I would assume it depends on the type of "car media center" your car has.
I have a Lexus NX200T and I had no issues pairing it with the car's system. Phone calls and Music work perfectly via BlueTooth. Only thing that my iPhone XS does that the Watch apparently doesn't is sending messages to the car.
Not sure what car you have, but I have a Lexus NX200T and while the pairing too a bit longer than with my iPhone XS, it did pair with my Apple Watch series 4. Phone calls and music work as expected (though I did not bother with syncing my contacts), but messages - which would be sent from the iPhone to the car - don't seem to work.

Not sure why it would work on your car and not mine. I’ve tried to get it to pair several times and while both the watch and the car can see each other they always fail to pair.
I would ideally like to leave my phone at home if going once short trips and just wear the watch. But, I really want the music to stream while I’m driving so need the pairing.

Surely it makes sense that people would want to pair a Bluetooth device with another Bluetooth device.
Unfortunately there is no way to know why, and we are not allowed to speculate on Apple's decisions in these forums.
