I don't see my phone number listed under FaceTime/iMessage settings - both in my phone and my mac. As a result, I'm not able to make phone calls directly from my mac.
If I try to call from mac, I keep getting the error 'Your iPhone must use the same iCloud and FaceTime account.'

Update: I found the solution for this problem. I had to recharge my mobile balance, so that there would be enough balance for the FaceTime activation SMS. (It costs 5 rupees in India)
Once I recharged, signed out of FaceTime, iMessage in my phone and signed in again, my phone number appeared finally!

Just checking since you didn't mention it - after adding the new number to your ID, did you sign out of Messages/FaceTime, restart, and sign back in?
Are you using the same Apple ID for iCloud and FaceTime on both the phone and the computer?
After adding your phone number to your Apple ID, did you wait 24 hours for the change to sync, then sign out of Messages/FaceTime, and then back in on the phone and the computer?

Yeah, I did that. Both on mac and phone. It didn't help.
Yeah, I'm using the same Apple ID on both phone and mac.
And yeah, it's been months since I added the number to my Apple ID. I had even contacted Apple Support a few months ago. They asked me to wait for 24 hours and I did but the problem did not get resolved.
Text messages forwarding is already enabled.
OK so I've been using my mac for 3 years now. But I got my phone only 6 months ago and I think I might've linked my phone number to the Apple ID only then. I have 2 other email IDs linked to my Apple ID. I can see all 3 email addresses listed in the FaceTime and Messages settings. Only the phone number is not listed.
