I can talk ok. If I try to change the phone to speaker, the screen goes black or blinks.

The built in proximity sensor should cause the phone screen to lock if you hold it up to your face (or hold your hand over the top of the phone). When you pull it away from your face, it should light up again. If it's not lighting up again when you pull it away from your face, there may be something wrong with the sensor. Contact Apple to have your phone evaluated.
During Phone Calls and some video calls, the screen will turn black if the front facing camera is blocked. It does this so you won't accidently press a button with your ear. If you need to see something on the screen during your call, uncover the front facing camera.

It's not covering or uncovering the camera the causes the screen to go dark. It's covering or uncovering the proximity sensor. Yes, they are right next to each other but they are not the same.
That's how it is designed. When you take your iPhone up to your ear to begin conversation the touch will be disabled.
That is what it is supposed to do. Whenever you put your ear up to the iPhone, the screen will go black. It is not an issue and its how the design is.
It could also be a case or screen protector that covers the sensor.
True, very true.
Be careful of how you're holding it. If your hand covers the proximity sensor (top center next to the camera), you'll cause the screen to go black.
