Incorrect Album Art on my iPhone 13
The album artwork for several albums is mixed up. The artwork is correct in iTunes, but I just did a sync to my phone and now things are messed up. Resyncing and restarting did not fix the problem.

The album artwork for several albums is mixed up. The artwork is correct in iTunes, but I just did a sync to my phone and now things are messed up. Resyncing and restarting did not fix the problem.

I deleted the Music app from my iphone 13, and deleted all of the music. Then deleted all of the backup files that i have stored on my iMac, and resynced the phone to find..... a whole load of the album art remains messed up, This is not a minor issue as I really use the album art to select music when I am using car-play, as it offers a much quicker recognition of the album than having to search by name. Now, there is added jeopardy when selecting music in the car as I HAVE To read the album title to be sure of what I'm going to get. From a cursory review it would seem that all music I have bought form the itunes store is fine, but anything I have synced from my CD collection over the years is now corrupted. By the way, on the iMac, the album art is correct - it's only an issue on the iphone.
Come on Apple - sort this out - you should be embarrassed that such a basic feature has been an issue for your customers for so long. We don't all want to only use streaming services for our music - some of us have collections which we would like to keep.
Very freaking annoying!! Was pushed into an iPhone 11 after my 6 would no longer support the OiS updates and a bunch of my apps quit working. Now I go to add some new music and this BS happens!!! Thanks Apple, thanks a lot.
I have over 30000 tracks on my iPhone and me live for my music. New iPhone all my music artwork got jumbled up. Assuming your music and artwork is backed up correctly on a laptop, mine was. Deleted all music from phone, once finished left it hooked up for 30 mins plus. Disconnected then reinstalled music from laptop back to phone, left it again hooked up for an hour after it had finished transfer. All was well! Time I left it hooked up was because I was doing other things, point is it’s possible there’s a lag, once music downloaded could be artwork is still working in background.
I have had the same issue on on 13 Pro. The concern with doing any SYNC, is I too have CDs ripped from years ago. At one time I got a free Apple Music free trial, so that I would give it a go. I actually liked it, and was going to subscribe, but the way it was set up I had to cancel and sign up again (I don't remember the exact reason now, but thats what support said I had to do). Well I immediately lost a LOT of my music. I escalated to 3rd level support and they just stopped trying and stopped working with me. I did recover most of it, but I know there is still some missing, and many of those were from CD's that I have lost or loaned, or even borrowed, so I won't get them back, without purchasing again that is. Well now when I do try to upload another CD and have to sync it to my phone, I run into the risk of losing other music. But what also happens as random albums with duplicate songs and the only way to fix that is manually deleting each duplicate. So I just live with the artwork NOT matching...which is a pain. Especially when trying to find an album while driving. When I ask Siri, it seems to want to go to Apple Music (which I did not re-subscribe to). I'm sure some of it is ME and not learning it, but frustrating for sure.
To correct the issue for my iPhone 13, I deleted the cache folder in my iTunes Music folder on my windows PC ( making sure I had a backup somewhere first). The I removed the music app from my phone. Reinstalled the music app onto my phone. Performed a sync and the issue has been resolved. I’m very happy now!
I have the iPhone SE (3rd Gen), and experiencing the same problem. I ripped all music from CD, have over 8,000 tracks on my phone. I am syncing from a PC. First all of the music was duplicated, then I wiped all stored music on the phone, re-uploaded and now all of the artwork is incorrect. It takes hours to upload all of the music and don't feel like re-wiping all of the music again. It's pretty bizarre.
I had the same issue after I upgraded my iPhone SE to iPhone 14 pro and first synced with iTunes. A good portion of my albums had the art all mixed up. I deleted the music app from my phone, re installed it - thus deleting all the music from the phone. Then I re-synced the phone with iTunes and BOOM! FIXED! What a relief!
THIS is the fix for 2022!! No other “solutions” I found on this forum or anywhere else on the web fixed the issue for me until I deleted all songs from my phone, deleted/reinstalled the Apple Music app, and resynced my iPhone in iTunes.

I realize this is an old thread, however I thought I'd reply to offer a solution for anyone still plagued by this. The issue does not appear to be caused by the phone, it's an issue with the desktop app. Specifically it's the cache folder on your Mac/PC and is one of my pet peeves in that applications do a terrible job cleaning up after themselves. Over time, things build up and become currupt and you simply need to delete the contents of the folder and it will automatically rebuild itself. On a Mac, open Finder>Users>{Select your ID}>Music>iTunes>Album Artwork>Cache and delete the contents of the Cache folder (Note: Do Not Delete the Cache folder itself, just the contents). I can't say for sure, but I think you will have to remove your play lists from your phone and re-synch. In my case, it was only one playlist that was jacked up, so it was easy and quick. I don't know where the cache is located on a windows based machine, however the process is similar. Good luck.
the album cover is now correct on my computer but can’t get it to correct iPhone it’s so frustrating when Apple breaks things and then doesn’t fix it. I have absolutely zero faith in deleting my library and expect all to correctly come back. Now I will spend hours reloading music I have downloaded from my cds.

Personally, what worked for me was deleting the music app from my phone, then reinstalling the app, and then syncing music from iTunes. Personally, I do not sync my entire library, I have certain playlists and parameters from what songs that I export. Again, this worked for me. Hopefully it will work for you.
I was able to fix the issue on my iPhone 13 Pro Max using suggestions from other posts .. here is what I posted previously that helped me ..

I waited on hold about an hour and got Apple support on the phone. Did a screen share in my iTunes (12.9.5.5)
It turns out, I was NOT syncing the device properly, and instead, I wsas using my 12 - year old method of dragging and dropping playlists. I guess THAT was the reason I was having issues. Without deleting the music app from my phone, instead, i synced the phone with the desired itunes playlists and the problem was resolved.
