I have noticed that after the iOS 16.4 upgrade on my iPhone 13 Pro Max that the Verizon 5g cellular network is not connecting. It is stuck using LTE. Prior to the upgrade I had 5g cellular network connection. I have restarted the phone, reset network settings, and checked that 5g auto is chooses in the cellular network settings. Nothing seems to bring back 5g.

Correct. Except, that 5G standalone toggle is completely missing. Is it still carrier's mistake?

Exactly my point. I have done all the troubleshooting and even a lodge a complaint on my carrier service but it’s actually nothing to do with them this issue is only related to ios software itself.
Even I’m having the issue after I updated to 16.4.1 my 5G is not working tried restarting flight mode and workplace is 15 miles from where I live. It doesn’t change to 5G.
If you're not on a 5G or VoLTE connection (meaning 4G or older) that's how things may work, there may not be bandwidth to handle voice and data simultaneously.
Phone doesn’t have 4g. It’s the 12 Max. My phone barely works on Wi-Fi . Nothing ever wants to load anymore unless i switch back & forth every 5mins
The iPhone 12 does support 4G (and even 3G.)
It may have been an interplay between iOS on some devices and carriers, simply because the vast majority of users had zero 5G issues with iOS 16.5.
Regardless, I'm happy 5G is back for you.
You had an issue with your particular device and it was apparently resolved.
