You cannot attach images to birthdays in the Contacts App.
You may be aware that when you add a birthday in the Contacts app the same is reflected in the Calendar App too.
You may also be aware that there are 4 types of Calendars on an iPhone, namely
Since you stopped at Chinese Birthday and have not explored further, you have this question. If you had tapped on the Chinese Birthday you would have got an answer.

My problem is this: I want to add a second birthday on the Gregorian calendar and it comes up with a Chinese calendar and the other choices are Hebrew and Islamic, the gregorian is absent.
in trying to enter a second birthday but it enters with the Chinese calendar. I tapped on the Chinese word and it changes to the Jewish calendar I do it again and it switches to the Islamic calendar. When I tap the Islamic the Gregorian calendar is missing.
