I have an open case with Apple Enterprise support on this issue for the last few weeks. They have pulled diagnostic logs from my phone and watch multiple times. They seem eager and willing to find the issue. In my experience in troubleshooting this I feel it is vehicle specific. I have a 2020 Hyundai Palisade with wired CarPlay that has the problem. My older Nissan with an aftermarket CarPlay receiver has no issues. For me, the issue occurs only when I cut the car off and open the door so the infotainment system then shuts off "before" I pull the lighting cable, about 90% of the way I do it. If I pull the cable from my phone before CarPlay shuts off I NEVER have the issue. I have tested and tested this so many times I've lost count. I think it has something to do with the way the vehicle is sending a disconnect or maybe not sending the disconnect command to the phone so perhaps the phone still think it's connected to CarPlay in some way. Rebooting the phone resets this. This would explain why it doesn't happen to everyone. For the wireless CarPlay users if you turn off WIFI before you shutoff your vehicle do you still have the issue? I think there have been several posts that this was proven true. For you wired folks like me, if you unplug the lightning cable before your CarPlay turns off does your watch ring normally every time? I am sending more diags to Apple once I get home today as requested by Apple Engineers. I am very hopeful we are getting to a resolution with this.
Yes, I have the issue with wi-fi only watch - no cellular.
I believe that setting is just used to allow the Watch to continue to stay connected to the iPhone while in Airplane mode. But curious to see if you have any luck with it.
Thanks.
i Would like a new series 6 since I have only had it for a month. I thought the bug would’ve been fixed by now in an update. I am disappointed because my cell service doesn’t work at all on the watch even if rebooted. I’ll call Apple support again to see. I didn’t know that other new watches were fine. Clearly it must be defective
Just an update.
After disconnecting from car play the watch doesn’t receive call notifications anymore. (This is after a factory reset on both the watch and phone with a new setup, not from backup)
as mentioned before my wife has the same setup and hers works after disconnecting from CarPlay.
hopefully apple sort this out as it’s annoying!
It's great to get the updates guys! Please keep us posted if you hear anything more! This is ongoing the past couple of months and I'm not holding out much hope either....
But at least there is a good few examples of the issue.
Is there anything I can do to raise this?
I've submitted it through feedback on the latest beta version but I don't know if I could do more?
Similar issues as above - BMW Wireless Carplay, iPhone 12 Pro Max and Apple Watch 4.

Maybe with iphone 12 you may want to contact support as some others.
With my "old" devices support has expired, and I'm not going to pay a cent for the new, until this is resolved.
So I also have submitted via the feedback app. Case is still "open", with no comments. Same as two other open case that I submitted in ..... 2015.
Today I realized, that after disconnecting of AppleCar (Honda e), my Macbook 12 (2017) is also without call's notification. I also found out that sound is off even it is on. Sometimes sound worked with standard simcard, esim was without sound. Iphone pro max 12 and apple watch 5. last ios.
Same here. Updated watch 6 and iPhone 12 still no cell incoming calls. Was on the phone last night and they did another analytics & improvements to share watch info with their engineers. So far, my new watch is completely dependent on having my phone with me at all time to get calls..rebooting phone doesn’t work
FYI - My latest email from apple

Hey ****,
No new information but I can setup a call if you like. My Hours are M-F 8-5 EST.
Apple is aware of the issue and is still investigating. We suggest that you keep your device up-to-date and report any change in the issue.

Mine never works for incoming calls or SMS messages. Even with a reboot. I will receive calls from FaceTime audio and get iMessages. There’s some problem with incoming calls that are using my cell number (not my Apple ID like FaceTime audio does). I can do outgoing calls so it isn’t a cellular problem
for as much as these devices cost, you would think that Apple would have found a solution to this issue by now. We shouldn’t have to restart our phones every single time after we connect to car play. I’m definitely frustrated. Thank you all for your input. Hopefully we will get a proper resolution to this issue soon.
I think you are right, but can you explain how you confirmed this? I tried to test it as well before and after getting in my car (CarPlay activates automatically). I called myself from another phone and my watch "rang." After returning from a trip in the car, I re-dialed myself and my watch "rang" again. But I did not answer either of those calls. If you could share how you tested it, I would like to try and replicate.
By any chance has anyone who has this problem also have a problem with Time To Leave notifications not working on calendar invites? This started happening at the same time when I bought my Apple Watch 6 (I upgraded to an iPhone 12 at the same time). I find it coincidental that both of these things started happening at the same time. And, I also did test invites before and after CarPlay activates and Time To Leave notifications seem to stop working afterwards.
Hi, I am working at home during the pandemic and driving just 2 or 3 times a week. The problem never happens when I am at home (after restarting the phone when I came home), but happens after I came from driving, not always but most of the time. Also everyone who posted here (including you) uses Carplay.
Thanks for responding. When I sent my reply I didn't realize I was on page 2 of a 10 page thread. i have since read all the responses and have an idea of how to replicate the issue. I too have been WFH during the pandemic but I found the issue to be occurring after a 5 minute drive to get take out. Fingers crossed IOS 14.5 fixes it for good. Stay safe!
I've been going insane over this and did not put two and two together regarding Wireless CarPlay in my BMW. I just erased my Watch and installed as new, also restored... went from iPhone X to 12 Pro Max thinking it would fix it, but it has not. It has been fixed by resetting my phone and watch for a while, but then that stopped correcting the problem. This has been happening for some time it looks like. It's hard to believe there has not been a fix yet.
It would work if I had a single story house with a garage next to it, but it is not possible for me to connect to my home wifi, which is 40 floors above 😂
It is incredible that Apple did not fix the problem for such a long time 😡
